The link below will take you to an online health care survey for members of Actors’ Equity, AFTRA and SAG.  All three unions are working together to address the health care needs of their many members who do not qualify for the union/producer health plans.  Data on members’ status and needs is critical to this effort. 

          The online survey, which takes less than ten minutes to complete, is comprised of questions regarding types of coverage, urgent needs, demographics, and personal/family status.  All information collected will remain strictly confidential, and members may skip a question if they find it too personal.  If you don't have Internet service, there are kiosks set up at designated union offices where you can access the Internet.  In the altertnative, members can obtain a hard copy of the survey at any union office, any AFTRA-SAG Federal Credit Union location in Los Angeles, at the Actors Federal Credit Union in New York, any office of The Actors Fund,  or at The Audition Space in New York.
